Medical transcription in the EHR era

Clinical documentation eats roughly 16 hours of a typical physician's week, and the documentation-to-patient ratio is the most consistently cited driver of physician burnout. Medical transcription used to mean dictating into a handheld recorder at end of day and shipping the tapes to an offshore transcription service that returned typed notes 24–72 hours later — a workflow that's now mostly extinct, replaced by either typing directly into the EHR (slow), real-time scribes (expensive), or modern asr">ASR systems integrated into the EHR (fast, but locked into the EHR vendor).

Where general-purpose AI transcription like ours fits is the gaps the integrated EHR systems leave open: recordings made off-platform that need to land in the chart. That's voice memos from a surgeon between cases, recorded patient interviews from a research study, telehealth visits where the EHR's built-in transcription failed, audio handoff notes between shifts, and dictated chart notes done via phone or smartwatch.

HIPAA and the practical privacy bar

HIPAA requires that protected health information (PHI) be handled by entities that have signed a Business Associate Agreement (BAA). Our standard service does not include a BAA — meaning you should not upload audio containing identified PHI to the consumer-tier service. Two practical paths for clinicians who want to use AI transcription:

1. De-identify before upload. Strip name, DOB, MRN, address from the recording before transcription. The medical content is still useful (clinical reasoning, differential, treatment plan) without the PHI. This works well for teaching cases, M&M conference notes, and case-discussion recordings.
2. Use the BAA-tier integration (contact us). Larger practices and hospital systems can request a BAA-covered tier with the appropriate safeguards. Available on annual contracts.

Either way: audio is deleted from our servers immediately after transcription, and we never train models on user data.

Where AI transcription saves clinician time

• Voice memos to chart notes. A surgeon dictates a 2-minute post-op note into Voice Memos on their iPhone between cases. Drop the .m4a (or M4A) file in here, get a structured transcript that pastes into the EHR free-text field.
• Recorded patient interviews for research. Phase IV studies often involve recorded interviews with patients about treatment experience. Transcribe once, then run our AI summary, key-quotes, or thematic-analysis features for IRB-friendly qualitative analysis.
• M&M and case-conference recordings. Morbidity and mortality conferences are often recorded for educational use. Transcribe to text once, then use the transcript as the basis for teaching notes, board prep, or case writeups for a journal.
• Telehealth visit recap. Patient gave consent to record? Transcribe the visit, generate a patient-friendly visit summary with our AI summary feature, send via the patient portal. Reduces the post-visit "what did the doctor say?" follow-up calls.
• Continuing medical education content. Recorded grand rounds, conference talks, podcast guest spots — convert to text, use the AI features to extract teaching points and CME-eligible summaries.

Accuracy on medical content

Medical vocabulary is the hardest test for general-purpose ASR. Drug names, anatomical terms, procedure codes, and Latin abbreviations are often out-of-distribution for the underlying language model. Realistic accuracy expectations:

Strong medical transcription specialists still beat general-purpose AI on the most technical content. The right question isn't "AI vs human" — it's "where does AI save you the first 80% of the work so the human reviewer can focus on the 20% that actually matters?"

FAQ for clinicians

Can I use this for chart notes in a HIPAA-compliant way? Not on the consumer tier without de-identifying the audio first. For HIPAA-covered workflows, contact us about the BAA tier.

Does it understand my EHR's templates (SOAP, H&P, discharge summary)? The transcript itself is structure-agnostic. After transcription, you can run our "Outline Notes" or "Q&A Split Page" AI features to reorganize the content into the section structure you want — but final formatting still goes into your EHR's smart-text or template macros.

What about non-English patient interviews? Whisper supports 50+ languages with strong accuracy. For multilingual clinics, this is one of the bigger time savers — transcribe in the patient's spoken language, then run our Translation feature to produce the English version for charting.

Will this replace my medical scribe? No, and you shouldn't want it to. A scribe contributes context, clarification questions, and EHR navigation that a one-shot transcript can't. AI transcription works best as a tool for asynchronous documentation and as a force-multiplier for scribe-supported workflows.
